POSITION SUMMARY:

The Home Health After Hours Registered Nurse (R.N.) will be responsible for the implementation of all health and teaching needs of the patient in the home environment as governed by the State of Alabama Nurse Practice Act.  The R.N. will act under the policies, procedures, goals, objectives and philosophies of the Home Health Department.  The RN will be responsible for the case management of the home health client, which includes but is not limited to, coordination of care for interdisciplinary team members, oversight of plan of care throughout cert period, coordination with physicians and other members of the health team.

Essential Duties and Responsibilities

  • Assesses patients assigned including past and present history, physical assessment and evaluation of the home and family environment.
  • Develops and implements the patient’s plan of care in collaboration with the patient, caregivers, physician and other members of the multidisciplinary home health team.
  • Documents and records neatly and concisely information in the paper and electronic medical record of the patient.  Provides accurate and pertinent information in the clinical record while noting patient changes and to whom these changes were reported and any changes in the patient plan of care.  Completes within 24 hours of the visit all forms, mileage, Daily Activity Records, electronic data entries, plans of care and physician’s verbal orders.
  •  Participates in a multidisciplinary approach to meet patient’s needs in a safe and caring manner while exhibiting a professional behavior for actions and job functions.
  • Participates in planned and continuous professional growth while identifying any developmental needs and completing annual orientation and competencies required.
  • Develops patient plan of care (POC/POT) in coordination with Physician orders.
  • Supervises the care of the LPN and Home Health Aide.
  • Documents nursing care and interventions.
  • Provides patient care, patient teaching and coordinates the care with other disciplines and team members with the assistance of the Clinical Supervisor.
  • Participates in the implementation of the Quality Improvement Plan/Performance Improvement Plan.
  • All duties performed during scheduled hours of 5:00 pm – 5:00 am.
  • Provides effective communication to patient/family, team members, physicians, and other health care professionals
  • Adheres to all company policies and procedures.
  • Adherence to and compliance with information systems security is everyone’s responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered.Report information systems security problems.
  • Provides effective communication to patient/family, team members, physicians, and other health care professionals

Minimum Qualifications:

1. Knowledge of basic math skills, with ability to read, write, speak and understand    

    English. Ability to understand/learn basic computer skills required.

2. Current CPR required.

 

Education/Licensing/Certification:

  • Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing
  • Current, unencumbered State License as a registered Nurse
  • Current Driver’s License
  • Current CPR, negative TB screen and Hepatitis consent/declination

    Experience:
     
  • Two years of experience as a registered nurse in a clinical care setting, home health preferred.

    Knowledge and Skills:
  • Nursing skills as defined as generally accepted standards of practice.
  • Good interpersonal skills.
