Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assumes the responsibility for coordination of care.


Essential Duties and Responsibilities:

·         Psychiatric RN will be responsible for complete medical and mental health status assessments and documentation upon admission

·         Psychiatric RN will be responsible for all psychiatric skilled nursing visits and case management of all psychiatric patients

·         Psychiatric RN will provide clinical directions to the interdisciplinary team, physician, psychiatrist, and family as indicated

·         Initial and ongoing comprehensive assessments of home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation (OASIS at appropriate time points), clinical record, case conference, team report, and evaluations

·         Implements/develops/documents the plan of care to ensure quality and continuity of care

·         Provides care utilizing infection control measures that protect both the staff and the patient (OSHA)

·         Supervises and provides clinical directions to the HHA/LVN to ensure quality and continuity of service provided.

·         Assures continuity of quality patient care delivered with appropriate documentation

·         Monitors assigned cases to ensure compliance with requirements of third party payors

·         Performs on-call responsibilities and provides on-call service to patients/families, as assigned

·         Provides effective communication to patient/family, team members, and other health care professionals

·         Adhere to all company policies and procedures.

·         Adherence to and compliance with information systems security is everyone’s responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered.  Report information systems security problems.


Non-Essential Duties and Responsibilities:

  • Perform other duties as assigned.


Minimum Qualifications:



·         Graduate of an accredited college/university, Associate or Baccalaureate School of nursing

·         Current  State License as a registered Nurse

·         Current Driver’s License

·         Current CPR, negative TB screen and Hepatitis consent/declination

  • Must have a letter on file with agency from the Medical Affairs Department stating that Psychiatric RN may make psychiatric home visits that may be billed to Medicare.

·         Must meet all education and experience requirements outlined in the “Home Health Psychiatric Care Local Coverage Determination” through Medicare’s medical affairs department.



·         Two years of experience as a registered nurse in a clinical care setting, home health preferred.


Knowledge and Skills:

·         Nursing skills as defined as generally accepted standards of practice.

·         Good interpersonal skills.
